In South East London, we deliver Out of Hours Palliative Care.
Working in partnership with a local hospice, we manage and deliver to the patient's home, urgent palliative care medication.
The Palliative Care Service brings together local hospice, EoL (End of Life) clinicians, acute hospitals, pharmacy and SELDOC. Clinical staff contact SELDOC directly, bypassing NHS 111 eliminating unnecessary steps requesting urgent medication. SELDOC liaises with the Acute Trust's pharmacy department to obtain the necessary medication and then delivers it to the patient's residence.
