We are a healthcare provider with GP-led primary care at our core. We deliver our services in collaboration with partners across the NHS landscape, including Hospital Trusts, Ambulance Services, GP practices, Federations, Primary Care Networks, Commissioners, social enterprises and private sector organisations. Many of our services are integrated with 111, and our highly trained workforce triage patients to ensure they receive the appropriate clinical care. As well as treating patients remotely, we offer face to face appointments outside normal hours and, where clinically necessary, we see people in their own homes. We look after approximately 4,000 people each week and every decision we make is focused on the best outcome for each patient. Throughout the COVID-19 pandemic, we rapidly mobilised new services and are continuing to adapt our service in line with NHS requirements and changing patient needs.
