Please complete the membership forms and return the forms to Stella Wifa at SELDOC (either by fax, post or in person), together with:
(a) Evidence of Principal status
(b) Evidence of MDU or other relevant insurance cover
(c) Annual GMC Retention Certificate
(d) A copy of your CV

In the event of an acute collapse or dire emergency, dial 999…
…The surgery is open from (give times)… For an appointment please ring (give surgery number)…
